About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. The Santa Clara Assisted Outpatient Treatment (AOT) program serves adults age 18+ diagnosed with a severe mental illness. AOT serves individuals who are high utilizers of mental health services and are considered to be high risk for psychiatric hospitalization, and/or have been incarcerated.

Requirements

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
• Valid and current driver’s license
• Personal vehicle insurance with your name listed as a driver
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
• Willingness to use your personal vehicle to attend meetings, etc.
