About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare FSP is an Intensive Outpatient (IOP) program which serves up to 170 MHSA and AB109 individuals with SMI who have not responded well to traditional outpatient behavioral health care. Clients referred to this program should expect to receive a comprehensive array of ACT services and supports.

Requirements

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
  • Valid and current driver’s license
  • Personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
