The Case Manager II - Eviction Prevention provides specialized rent-related housing stabilization support to Minneapolis Public Housing Authority (MPHA) residents across Agate Housing and Services' Housing Stability program. The position leads direct assistance with past-due rent, repayment agreements, and related financial barriers to continued tenancy, coordinating with residents, MPHA property management, assigned Case Managers, and Program Managers to identify practical steps toward housing retention. The position also supports staff in initiating and implementing housing loss prevention plans and maintains centralized tracking of all plans across MPHA properties, including actions, deadlines, follow-up, and housing retention or transition outcomes. It assists with failed inspection follow-up and voluntary mutual lease terminations when appropriate, including resident-centered transition planning and connection to alternative housing resources. Assigned Case Managers retain their ongoing resident relationships; this position provides direct intervention for selected situations rather than taking over every housing loss prevention plan.
