Provides coordination in all aspects of home-based therapeutic service treatment plans and P.A.S.S. plans for a specified caseload in compliance with EOHHS & MCO standards. Provides general training to staff and families related to policies, procedures, and forms as they pertain to the HBTS and P.A.S.S. programs. Maintains ongoing communication via telephone calls, documentation, and attendance at meetings with all relevant parties involved with the specific child. Schedules and facilitates team meetings as applicable, including family members, therapists, consultants, clinical supervisor, school personnel, and CEDARR clinician or family service coordinator. Ensures child and family-centered involvement throughout all aspects of plan development, treatment, and follow-up.
