About The Position

This position is responsible for developing, coordinating, and monitoring each individual’s programs related to the Individual Support Plan. The Case Manager I will write Individual Support Plans (ISP) in ICF programs, review and communicate corrections for ISPs written by Independent Support Coordinators in Medicaid Waiver programs, and write Staff Instructions for all individuals. This role also involves coordinating the development of data sheets, training staff on ISP implementation, monitoring ISP program effectiveness, and writing monthly reviews. The Case Manager I will serve as the contact for Independent Support Coordinators in Medicaid Waiver programs and perform other requested functions.

Requirements

• Bachelors Degree in Human Services or related field.
• At least one year experience with individuals with Developmental Disabilities.
• Ability to be insured as a vehicle driver by agency's insurance program.
• A valid driver's License.
• Good knowledge of basic English sufficient to read instructions, keep records and write reports.
• Successful completion of background and reference check.
• Successful completion of a drug screen.

Responsibilities

• Write the Individual Support Plans (ISP) in the ICF programs.
• Review Individual Support Plans written by Independent Support Coordinators and communicate needed corrections to the ISC in the Medicaid Waiver programs.
• Write Staff Instructions for all individuals.
• Coordinate the development of data sheets that are needed to implement the ISP and other care plans.
• Train staff on carrying out the ISP for all individuals.
• Monitor the effectiveness of the ISP programs through communication with shift supervisor and on-site observation. Coordinate any changes that are indicated.
• Write Monthly Reviews for all individuals and coordinate any changes that are indicated.
• Be the contact for Independent Support Coordinators in the Medicaid Waiver programs.
• Perform other functions as requested.
