About The Position

The Coordinated Entry Case Manager is responsible for completing and entering data into the Homeless Management Information System (HMIS) and completing Coordinated Entry Assessments to ensure a high level of data quality across the system. This role also serves as a liaison with consumers and homeless service providers.

Requirements

• Proficiency in a range of computer skills
• High School diploma required
• Experience with the homeless population
• Excellent Communication and Interpersonal skills
• Ability to work under supervision but also independently

Nice To Haves

• College degree in social sciences or a related field preferred

Responsibilities

• Enter client data in the LSNDC HMIS Data Base
• Work with System Administrator in generating required reports
• Develop expertise in using the Advanced Reporting Tool
• Complete program intakes forms
• Refer consumers to eligible services
• Establish a rapport to engage consumers to services
• Report any significant concerns to Supervisor
• Maintain training requirements
• Develop and maintain a comprehensive housing and resource database
• Maintain assigned program records
• Projects and duties as assigned
