Case Manager I - Targeted Case Management (Metro County)
About The Position
People Incorporated Mental Health Services is seeking a Case Manager I to work on our Targeted Case Management (Metro County) team. This role primarily works with clients diagnosed with chronic mental illness or other chronic health conditions. The goal is to connect people with services through developing person-centered support plans that help them live as independently as possible. Case Managers develop long-term relationships with their clients and operate autonomously, creating their own schedules. This is a full-time position, Monday-Friday, with business hours of 8am-4:30pm, located in Metro County, Minnesota, with a hybrid work arrangement. A hiring bonus of $1000 is offered to external candidates. Requirements
- Bachelors Degree in the behavioral sciences or related field (including but not limited to social work, psychology, or nursing from an accredited college or university)
- If no Bachelors Degree in a behavioral science or related field, the employee must meet one of the following: Have 3 or 4 years experience as a case manager aide; OR Be an RN without a Bachelors Degree and have 3 years or more combined specialized training in psychiatry and work experience consisting of community interaction and involvement or community discharge planning in a mental health setting.
Responsibilities
- Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
- Provide general knowledge and guidance on assisting clients in the multiple phases of applying for and receiving services and benefits.
- Attend bi-annual meetings with clients to monitor services.
- Develop and maintain a collaborative relationship with clients, serving as a role model by demonstrating positive, proactive, and respectful communications.
- Assess client needs and ensure continuity of care, which may include outreach and consultation with other partners and providers.
- Maintain regular contact with collaborative partners and make referrals to professionals, schools, and community agencies as needed.
- Make referrals and authorize services based on needs.
- Maintain records and document all coordination activities.
- Document all interventions, interactions, and relevant information to the treatment plan consistent with professional and organizational standards.
