Case Manager I/II
About The Position
This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet consumer needs, and serving as a Qualified Intellectual Disabilities Professional (QIDP) while providing monthly program monitoring. The position requires compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. This is a position in the Oklahoma Civil Service.
Requirements
- All New Employees will complete the Case Manager Academy Training Program.
- Case Manager - Level I: Bachelor's degree in a human services field and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field.
- Case Manager - Level I: OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities.
- Case Manager - Level II: Bachelor's degree in a human services field and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field.
- Case Manager - Level II: OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities.
- Combination of education and experience is acceptable.
- For purposes of the Case Manager job family “a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.
- Must possess a valid driver's license.
- Must maintain required car insurance.
Responsibilities
