This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet the consumer's needs, and serving as a Qualified Intellectual Disabilities Professional (QIDP) with monthly program monitoring. The position requires compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. Travel is extensive, and a valid driver's license with required car insurance is necessary.
