We’re looking for a dedicated Case Manager I – Family Support to partner with families and individuals with developmental and intellectual disabilities to build meaningful lives in their communities. If you’re passionate about advocacy, connection, and helping people thrive where they live, this could be the role for you. As a Case Manager I, you will assist in coordinating Family Support services alongside families, team members, and the Department of Developmental Services (DDS). You’ll help create and implement Family Support Plans, connect families to resources, and maintain consistent communication through regular home visits and outreach. Your work will focus on two key outcomes: Building positive and meaningful social connections, and Supporting access to meaningful work and daily activities.
