About The Position

The Healthy Start Case Manager plays a vital role in coordinating and implementing comprehensive case management services for pregnant women residing in the city of Syracuse. This includes conducting assessments, developing service plans, monitoring services, and providing necessary follow-up. They ensure regular face-to-face contact with clients, maintain confidentiality, and offer screening services for domestic violence and perinatal depression. The case manager also provides health education on various topics, facilitates prenatal care appointments, collaborates with relevant agencies, and offers referrals to community resources. Additionally, they are responsible for accurate documentation, maintaining case files, and attending regular meetings with their supervisor for guidance, support, and training.

Requirements

  • Associates degree in Human Services or related field with 2 years of case management experience, or equivalent combination of education and experience
  • Must meet all health standards for health department personnel as a subcontracting agency of the Onondaga County Health Department (OCHD)
  • Proficiency in utilizing electronic database systems
  • Strong proficiency in MS Outlook and MS Office
  • Excellent customer service skills
  • Effective oral and written communication skills
  • Valid N.Y.S. Driver's License
  • Registered & Insured Vehicle
  • N.Y.S. Child Abuse Registry clearance

  • Experience working with expectant mothers and parents of children under two years of age
