Requirements

Associates degree in Human Services or related field with 2 years of case management experience, or equivalent combination of education and experience.
Must meet all health standards for health department personnel as a subcontracting agency of the Onondaga County Health Department (OCHD).
Proficiency in utilizing electronic database systems.
Strong proficiency in MS Outlook and MS Office.
Excellent customer service skills.
Effective oral and written communication skills.
Valid N.Y.S. Driver's License.
Registered & Insured Vehicle.
N.Y.S. Child Abuse Registry clearance.

Nice To Haves

Experience in facilitating educational groups with vulnerable populations.
Experience working with expectant mothers and parents of children under two years of age.

Responsibilities

Coordinate and implement comprehensive case management services for pregnant women residing in the city of Syracuse.
Conduct assessments.
Develop service plans.
Monitor services.
Provide necessary follow-up.
Ensure regular face-to-face contact with clients.
Maintain confidentiality.
Offer screening services for domestic violence and perinatal depression.
Provide health education on various topics.
Facilitate prenatal care appointments.
Collaborate with relevant agencies.
Offer referrals to community resources.
Accurate documentation.
Maintain case files.
Attend regular meetings with their supervisor for guidance, support, and training.
