Requirements

A bachelor’s degree in social work or a related field, is required.
Must be med-aid trained
Familiar with electronic Medication Administration Record (MAR)
Must be at least 21 years of age
Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.

Nice To Haves

Bi-lingual is a preferred but not required.

Responsibilities

Developing client-centered case plans
Providing ongoing assessment
Assisting clients in accessing community resources to meet their needs
Maintaining accurate case documentation
Building collaborative relationships with community partners and service providers
Coordinating medical, dental, and vision appointments
Ensuring annual exams are completed
Assisting with Medicaid eligibility
Supporting medication management processes in collaboration with nursing staff and pharmacy providers
Obtaining medical records and treatment consents
Reviewing Medication Administration Records (MARs) for accuracy
Transporting clients to appointments as needed
Participating in the on-call rotation system
