About The Position

Catholic Charities Diocese of Venice, Inc. provides services to people of all races, backgrounds and beliefs. We offer a variety of housing and social service programs to individuals, families and communities across ten counties in Southwest Florida.

Requirements

  • Minimum High School Diploma or equivalent required
  • Bilingual in English/ Spanish required.
  • Well-developed written and verbal communication skills.
  • Ability to work well with culturally diverse staff and clients.
  • Must have reliable, adequate and available transportation.
  • The employee must provide a copy of their valid Driver’s license.
  • Excellent computer skills, especially with all Microsoft products (Word, Excel, Outlook, etc.).

Nice To Haves

  • Bachelors degree preferred.

Responsibilities

  • Ensures timely provision of case management services to eligible and enrolled participants.
  • The ability to search for and obtain needs for participants.
  • Effective development of a service plan to meet the needs of clients.
  • The ability to efficiently complete all relevant documentation and paperwork for all enrolled participants.
  • Comprehensive recording of service delivery for reporting and compliance purposes.
  • Provide advocacy and hands on services to all program participants.
  • Develop proficiency regarding the use of the agency electronic record system (ClientTrack).
  • The capacity to identify areas of need for participants and complete referrals to other services as needed.
  • Attending and actively participate in community meetings related to the homeless population and housing services.
  • Develop an effective working relationship with relevant agencies throughout the County.
  • Oversee and provide services to a full caseload of enrolled participants.
  • Other duties as assigned.
