About The Position

Provide case management and placement support for children and young people in foster care. This role involves conducting regular visits, phone contact, and care team meetings, as well as preparing various reports including court reports, case plans, quarterly reports, and end of placement reviews. The Case Manager will develop and implement care plans aligned with the Looking After Children (LAC) Framework, complete all required LAC documentation, and identify and respond to risk, incidents, and child protection concerns in a timely manner. Additionally, the role includes providing crisis intervention and support to carers and young people, liaising and collaborating with DFFH/Child Protection, schools, health services, and other providers, and completing Cultural Support Plans.
