About The Position

Adapt Integrated Health Care is looking for a qualified Case Manager - Forensics Community Navigator to join their team! This position provides community-based services, helping to support individuals with significant mental health issues in achieving their recovery goals. Specifically, this position works as part of a two-person team to provide recovery-oriented support to individuals under Aid and Assist who are preparing to transition from higher levels of care including Oregon State Hospital and who may already be transitioned to community-based care. This position is funded through a state award with an anticipated duration of approximately 18 months. Continued employment beyond this period is not guaranteed.

Requirements

  • Bachelor’s degree and two years’ experience in a social service and/or mental health setting or a satisfactory equivalent combination of education, experience, and training.
  • Must possess a valid Oregon Driver’s License.
  • Must be eligible to register as a QMHA through MHACBO.

Responsibilities

  • Provide support, education and intensive case management to clients to assist them in achieving their recovery-oriented goals beginning 3 months prior to discharge from Oregon State Hospital (OSH); up to 6 months providing community-based support.
  • Collaborate and coordinate with OSH staff while participating in treatment team meetings and discharge planning.
  • Maintain strength-based approach while utilizing motivational interviewing skills and providing stage appropriate interventions. Including tapering service intensity as client’s connect with long-term community care.
  • Implement approach to utilize and enhance consumers’ natural supports.
  • Coordinate work with involved family, caregivers, medical, psychiatric and other providers.
  • Assist consumers in accessing available benefits and services including housing, employment, SNAP, ID, SSD, etc.
  • Help facilitate coordination of intake and assessment, to connect and identify community-based services including services that can be started prior to discharge.
  • Continuously evaluate health and safety issues and implement appropriate crisis planning. Assist consumers in self-directing a crisis plan.
  • Complete regular and required reports/data tracking and submit OHA as required.
  • Work effectively within a multidisciplinary team.
  • Provides clinical documentation to meet State, Federal, Regional and departmental standards and requirements which includes: Developing recovery-oriented treatment plans with measurable and objective goals. Completing clear, organized, timely progress notes.
  • Learns/utilizes computer medical record system; including use of newer office technologies.
  • Maintain all applicable professional, legal and ethical standards, including confidentiality, dual relations, and informed consent.
  • Maintain productivity standards set by departmental policy and carry out other instructions from supervisor and/or Director.
