Adapt Integrated Health Care is looking for a qualified Case Manager to join their Forensic Mental Health team! The Forensic Case Manager assists in the development and maintenance of a healthy, supportive environment to make provision of effective therapeutic interventions possible within the community. The Case Manager works with individuals diagnosed with mental illness who are involved in the criminal justice system to promote recovery, independence and successful community living, by or through (a) Communication of hope, and promotion of emotional, behavioral and psychological growth through persistent efforts to attain individual goals; (b) The promotion of skills and knowledge to help individuals effectively manage their mental health concerns and develop a sense of hope and a sense of self that is not illness dominated; and (c) Provision of a humane service environment that affords reasonable protection from harm including re-traumatization. Provides case management, skills training and mentoring services to individuals with mental illness within the community and outside of Adapt Integrated Health Care offices. Provision of treatment services are informed by the Individual Service and Support Plan (ISSP) and clinically supervised by the primary clinician. Services are provided within a program for the Mental Health Division of the Health and Social Services Department in consultation with the treatment team. This classification provides advocacy and linkage in planning, monitoring, coordination, and implementation of the treatment interventions as identified in the ISSP. In addition, Case Manager’s assist individuals in problem-solving to reduce barriers to recovery and support the development of resources within the community while coordinating services with other community and treatment providers. Case Manager’s provide a variety of treatment services including individual and group skills training and development; case management services; coordination and monitoring daily structured activities of clients; performs crisis intervention as necessary under the guidance of the Forensic Mental Health Program Coordinator (QMHP) and/or supervisor; provides consultation and training; facilitates communication and correspondence with other Adapt Integrated Health Care staff, community partners, other treatment providers, peers, and all others as needed. This classification coordinates and collaborates with a multi-disciplinary treatment team and is responsible for work performed in assigned specialized program area.
