Requirements

  • 18 years of age or older
  • Ability to obtain a current CPR/FA certification
  • Ability to obtain fingerprint clearance
  • Ability to clear a DES background check
  • Ability to obtain a current T.B. clearance card
  • Ability to work in a team environment
  • Ability to handle difficult situations with clients/families
  • Ability to work a flexible schedule
  • Ability to learn and operate UCPSA program software
  • Ability to work independently
  • Ability to lift and transfer clients (up to 50 lbs)
  • Own/have regular access to and be able to operate a motor vehicle for the purpose of visiting and transporting clients, attending meetings outside of the office, etc. Proof of current registration and insurance of the vehicle must be kept on file with UCPSA
  • At least one year of direct care experience, personally or professionally, a degree in a related field, and/or case management experience
  • At least one year of teaching or related experience (paraprofessional, habilitation provider, registered behavior technician etc.) required

Responsibilities

  • Coordinate in-home and community-based services for the elderly and people living with all types of disabilities.
  • Work Monday-Friday 8a-5p, with occasional requirement to work a slightly different schedule to accommodate client needs, including direct care shifts and some evenings/weekends.
  • Perform on-call duties at least 4-5 times per year, including answering and managing emergency phone calls from 7p-7a M-F and all day Sat and Sun, which may also include working direct care shifts.
  • Maintain and grow a caseload.
  • Schedule shifts for clients and direct care staff.
  • Orient and train direct care staff with clients.
  • Monitor direct care staff performance.
  • Hold staff meetings as needed and attend weekly department meetings.
  • Conduct intake of new clients.
  • Meet with clients/families in their home minimally every quarter.
  • Attend meetings with other agencies involved in a client's care.
  • Ensure that all paperwork in client files is updated regularly.
  • Document all significant conversations and/or correspondences with staff and clients.
  • Maintain client and agency confidentiality at all times.
