About The Position

Developmental Pathways (DP) is seeking Enrollment Case Managers to join our Case Management Team! As an Enrollment Case Manager, you’ll play a vital role in delivering compassionate, person-centered service and support individuals to enroll in Medicaid Waiver and State General Fund services. This dynamic, hybrid role offers a fast-paced and rewarding experience where you help weave together services that empower the lives of individuals with disabilities. The schedule offers a 4-day, 40-hour workweek (Monday–Thursday or Tuesday–Friday) with the flexibility to work remotely, in-office, and in the field. This position requires in-person presence to support essential responsibilities such as service planning, assessments, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas, and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.

Requirements

Bachelor’s Degree; or Five (5) years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities; or Some combination of education and relevant experience appropriate to the requirements of the position.
Valid Colorado motor vehicle license and provide proof of motor vehicle insurance.
Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years.
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Responsibilities

Conducting in-home visits to complete assessments, connect individuals to services, and facilitate service plan meetings.
Engaging with families and providers in the community to build trust and foster strong partnerships.
Collaborating at our Aurora office, sharing insights and strategies with your team and other departments to enhance service delivery.
Working from home, which includes hosting virtual Zoom meetings with individuals and their teams, documenting and coordinating services, and participating in virtual office hours, team meetings, and more.
Driving between locations, spending time in the community and at your home or chosen remote workspace.
Act as a system navigator to assess the overall needs of an individual and assist with the exploration of programs, long-term services, supports, and other community resources to meet assessed needs.
Ensure timely response to internal and external referrals for individuals pursuing programmatic supports.
Connect identified needs with services and supports available to ensure a person-centered approach to support plan development; ensure eligibility for desired services is secured and maintained.
Serve as subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases.
Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies.
Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines.
Complete required documentation related to monitoring activities; evaluate need and effectiveness of services requested and delivered.
Schedule, facilitate, document, and complete follow-up for all required and ad-hoc meetings; disseminate updates to the necessary parties.
Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties, including, but not limited to providers, individuals, and family members.
Maintain knowledge of state/ federal regulations, policies, and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs, and other community resources.
Maintain a professional and ethical manner in all interactions and meet performance quality, customer service, and coordination standards as assigned by department leadership; maintain appropriate boundaries.
Assist with Intake duties as needed.
