This position is responsible for designing and developing plans for participants, including determining and assigning work activities, educational and training programs. The role involves conducting assessments for barriers and skills, determining eligibility for supportive services, and managing participant progress towards goals. The Case Manager will also monitor client progress, maintain case files, and stay updated on program procedures and community resources. Communication with employment organizations, educational programs, and clients in employment is crucial, as is ensuring all performance metrics are met. The role requires the ability to perform case management duties in-person and on-site.
