About The Position

The ELECT Case Manager is responsible for providing school-based case management services, including home visits, ongoing assessment, service plan development/review, referral connection and follow-up, advocacy, data collection and entry, and submission of programmatic and statistical reports. This position is primarily located in assigned School District of Philadelphia schools. A sincere interest in working with and supporting teen parents, their children, and their overall health and development is essential. The role requires facilitating and maintaining contacts with fellow ELECT Program team members, other Congreso departments, and assigned school staff. Knowledge of external community resources is necessary. This position involves handling sensitive Congreso information with integrity and professionalism and representing Congreso professionally in regular contact with the community. The position reports to the ELECT Program Manager and is an ONSITE position. The ELECT Case Manager applies the principles of the organization’s signature service delivery approach, Primary Client Model (PCM™), promoting a positive employee/client-focused experience that is appreciative of individual strengths, values, and needs, and uses data to inform decision-making. Attention is given to compliance, quality assurance, and best practices. Requirements

  • Basic understanding of Congreso’s mission, values, DEI&B vision, Mission2Impact, Primary Client Model, programs, and services.
  • Demonstrates knowledge and understanding of the community needs and demographics.
  • Ability to understand, acknowledge, and assess client needs to provide a timely, trauma informed approach in service.
  • Ensures the appropriate handling of sensitive information and understanding for issues such as confidentiality, domestic abuse, child abuse, and mandated reporter requirements.
  • Adheres to all Congreso, departmental, programmatic, and security policies, procedures, and guidelines.
  • Attends all Congreso trainings and in-services as required, including relevant professional groups to maintain professional, requisite knowledge and licensure.
  • Attends, participates, and facilitates relevant staff meetings to promote communication and execution of goals.
  • Ability to effectively use standard office equipment, including a variety of common software programs such as Office365, UNIDAD, and other customized databases.
  • Follows through on assignments, works in a timely manner, within established deadlines, and completes special projects specific to the function of the department, as needed or as directed by supervisor.
  • Demonstrates ability to work independently and collaboratively with diverse groups, in a compassionate, courteous, and respectful manner that fosters an inclusive environment.
  • Strong written and verbal communication skills including effective and respectful communication with individuals and groups.
  • Valid driver’s license required.
  • Mandated reporter certification required, can be obtained upon hire.

Nice To Haves

  • Bachelor’s degree in Social Work, Psychology, Human Services, Education or related field preferred.
  • Bilingual (Spanish/English) written and verbal skills a plus.

Responsibilities

  • Represents Congreso’s mission, values, and commitment to diversity, equity, inclusion, and belonging and will be an ambassador of our work.
  • Carry a caseload of a max of 20-25 teen mothers or fathers as assigned by Supervisor.
  • Conduct home visits weekly/biweekly, monthly or as assigned by supervisor.
  • Develop a goal plan with each teen for frequent and periodic monitoring and assessment.
  • Provide health, parenting and prevention education to program participants in their own homes, in the community, and in the assigned school site.
  • Provide and assist clients with health options, referrals and information about health choices providing advocacy as needed.
  • Engage other family members in the intervention plan of clients services and during the health/parent education sessions when appropriate.
  • Assist clients with crisis intervention services.
  • Maintain accurate and up-to-date client files and documentation.
  • Maintain strong relationship and communication with assigned school sites.
  • Network with area organizations, hospitals, daycare centers, clinics, etc. in order to identify and connect clients with appropriate community resources.
  • Conduct presentations and outreach activities to identify and recruit teen parents to participate in ELECT activities.
  • Work with the ELECT team to develop and facilitate educational and/or health-related workshops to clients.
  • Refer and connect teen parents with ELECT’s Enhancing Teen Parenting Skills services.
  • Implement Congreso’s holistic approach to client services by connecting families to multiple services within Congreso as needed to increase family supports.
  • Assist in the coordination and facilitation of Parenting Classes and other related activities as needed.
  • Maintain proper documentation of client records according to goal plan, agency and funding guidelines.
  • Submit weekly case notes and other funder-related reporting requirements into the funder database.
  • Regularly attend and actively participate in staff meetings, training sessions, forums etc. and attend bi-weekly supervisory meetings.
  • Comply with all clients and program confidentiality regulations mandated by federal, state, city and agency regulations.
