About The Position

The Case Manager - Domestic Violence Programs will be responsible for assessing and evaluating residents for admission, engaging them in treatment processes through individual and group counseling under clinical supervision. This role involves developing and maintaining housing referral resources, assisting residents in securing permanent housing, and referring them to community-based resources and aftercare services. The Case Manager will also assess and address residents' vocational and educational needs, liaise with H.R.A. Public Assistance and Medicaid Administrators, and participate in training seminars to enhance clinical skills. Responsibilities include attending supervision and case conferences, functioning independently during facility coverage, assessing client safety and well-being, making appointments for physical exams, developing client goal contracts, responding to hotline calls, facilitating psychoeducational groups, coordinating on-site services, identifying concrete needs, and performing other duties as assigned.

Requirements

  • High school diploma
  • Two (2) years of relevant training and experience, including experience working with victims of domestic violence; or Bachelor's degree in health or human services.
  • Two (2) years of crisis counseling experience preferred in a residential shelter for victims of domestic violence, and experience and demonstrated skills in group counseling.
  • Training and experience in assessing the vocational and educational needs and aptitudes of clients.
  • Thorough familiarity and success with engaging clients.
  • Experience assisting eligible clients through the process of obtaining Public Assistance, Medicaid and other benefits.
  • Knowledgeable and sensitive to domestic violence issues.
  • Demonstrated sensitivity and experience with and/or knowledge of domestic violence within and against disenfranchised communities including individuals whom identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, the elderly and individuals with mental health issues.
  • Demonstrated ability in delivering client services including: communication and listening skills, understanding of human interaction and personality development, the ability to write and record case documentation, and demonstrated skills in group counseling.
  • Computer-literate with knowledge of Microsoft Office Applications.
  • Ability to be flexible of schedule as needed.
  • Excellent written and verbal skills.

Responsibilities

  • Conduct intake interviews to assess and evaluate residents for admission.
  • Engage residents in initial and ongoing treatment processes through individual and group counseling, under clinical supervision.
  • Develop and maintain housing referral resources and assist residents in locating and securing permanent housing.
  • Assist residents in referral and placement in community-based resources, including aftercare services.
  • Assess residents’ vocational and educational needs, aptitudes, and interests, and develop placement and follow-up plans.
  • Liaise with H.R.A. Public Assistance and Medicaid Administrators and Workers to assist eligible residents in securing benefits.
  • Participate in training seminars to enhance and develop clinical skills.
  • Attend weekly individual case supervision, group supervision, monthly case conferences, and staff meetings.
  • Function independently in time of facility coverage, including responsibility for appropriate action in crisis situations.
  • Regularly assess the safety and well-being of clients.
  • Make appointments for residents’ physical exams.
  • Develop with the resident, a contract of mutually agreed goals and objectives.
  • Respond to Hotline calls as appropriate.
  • Facilitate psycho educational groups.
  • Coordinate on-site services such as for child care, child therapy and recreation.
  • Identify concrete needs and provide on an as needed basis.
  • Perform other duties as assigned by the Social Work Supervisor.
