About The Position

Under limited direction, coordinates the plan of care for the patient and family through the continuum of care, in accordance with the CVRMC Clinical Resource Management Plan.

Requirements

• Current Arizona RN license or Clinical Social Work certificate in good standing.
• At least 5 years of clinical experience.
• Ability to interact effectively with co-workers, patients/family, physicians, and other hospital personnel.
• Ability to perform effectively under physical and mental stress, maintaining concentration during that duration.
• Maintains current Case Management certification (once acquired).
• Maintains CPR.

Nice To Haves

• BSN or CSW preferred.
• Recent case management experience in a healthcare setting preferred.
• Certification in Case Management by a recognized national Board preferred, and will be required within three years of hire.
