This is a full-time, 40-hour per week position that combines 20 hours of Case Management with 20 hours of Direct Support Professional (DSP) duties. The role is expected to transition to full-time case management over time. The qualified candidate will work effectively as a team member, be resourceful with excellent communication skills, and possess flexibility to meet the needs and mitigate challenges in the lives of the individuals supported. Responsibilities include assisting individuals in identifying and addressing their needs, supporting activities of independent living (money management, household management, medication management, health maintenance, and appointment follow-up), helping individuals access public benefits (food stamps, Mass Health, Medicare, Social Security, housing, fuel assistance), facilitating community connections (Best Buddies, Special Olympics, Senior center, YMCA), supervising and working with direct care staff, and collaborating with agencies such as DDS, DCF, MHA, and BHN.
