The Case Manager is responsible for the overall coordination of care, along with some responsibility for the overall and long-term elements of the program. This role involves developing Individual Service Plans (ISPs) with clients, families, funding agency representatives, and treatment team members. The Case Manager facilitates ISP/Individual Treatment Plan meetings and Interdisciplinary Team Meetings, ensuring timely completion of all necessary documentation, including progress summaries, progress notes, and monthly/quarterly updates. This position serves as a primary contact for families, agencies, and other professionals involved in clients' care, maintaining regular contact with clients and coordinating services among providers to ensure continuity of care. The Case Manager also attends professional meetings, schedules and transports clients to appointments, advocates for clients' safety and welfare, and coordinates the admission and discharge process. They communicate regularly with management to ensure treatment goals are implemented in the residence and provide updates on any changes. The role requires practicing safe crisis prevention and intervention strategies, providing coverage in emergency situations, and assuming on-call duties as assigned. Participation in required in-service training for continuing professional development is expected, and the Case Manager is responsible for the overall maintenance and quality of clients' residential files.
