In partnership with the Department of Youth & Community Development (DYCD), Housing Works launched the CRED Program, a new specialized 24-week paid training program for justice-involved and justice-impacted individuals ages 25 – 40. The program provides case management, connection to wraparound health services, industry recognized credential trainings, paid internships and employment placement and retention support. Reporting to the Program Manager, the Case Manager will be responsible for, but not limited to, conducting screenings and intake assessments, co-developing service plans, connecting individuals to wraparound healthcare services, assisting with benefits and entitlements enrollment, providing program retention support and utilizing the agency and funder’s electronic records systems to document client engagement and program outcomes. This is an ideal opportunity for a reentry services professional with direct-care experience who is looking to develop skills in the area of care coordination and workforce development, and who is enthusiastic about working alongside an interdisciplinary team of like-minded professionals committed to improving outcomes for individuals and communities impacted by the criminal legal system.
