Coordinates the needs of clients and family members with available resources, providing client advocacy and assisting clients in accessing community resources for independent functioning. This role involves helping clients, individually and in groups, develop independent living skills, social skills, and engage in community activities. A key responsibility is assisting clients in creating individualized recovery plans focused on personal and interactive problems, defining goals, and planning actions based on their interests, abilities, and needs. The Case Manager establishes rapport with clients, assesses their needs, organizes and delivers face-to-face services, and follows up on outcomes. Services may include individual or group rehabilitation to address symptoms through education and skills practice for daily living and community integration. The role requires adherence to agency policies, CARF and DMHSAS standards, confidentiality regulations, and funding source requirements. A strong emphasis is placed on supporting a recovery-focused, welcoming, and trauma-informed service environment. The position involves attending annual core competencies training, maintaining professional working relationships, and effectively collaborating within a team. Responsibilities also include monitoring client eligibility for various benefits, maintaining up-to-date client information, and ensuring strict confidentiality. A significant portion of the caseload (at least 80%) will involve providing clinical services, utilizing Collaborative/Therapeutic Documentation, and completing all documentation on the same day of service. The Case Manager is expected to seek supervisory assistance for training and skill enhancement, interact with other professionals, and may be required to work evenings or weekends, and provide services in community settings. Client transportation using a personal vehicle or agency van/bus is also a requirement.
