About The Position

Lutheran Social Services of Illinois (LSSI), a leading not-for-profit organization, is seeking a full-time CST Case Manager for its Community Support Team. This role involves providing comprehensive, whole-person services to clients in mental health case management programs. Responsibilities include conducting 1:1 meetings, offering case management assistance, and utilizing evidence-informed interventions focused on behavioral, emotional, and social skill development. The Case Manager will help clients achieve their goals, collaborate with internal and external treatment professionals, and work with community partners. The position operates Monday through Friday.

Requirements

  • High school diploma or equivalent required.
  • Demonstrated proficiency in Electronic Health Record documentation and review required.
  • Valid driver’s license, in good standing for the state of residency required.
  • Access to reliable transportation required.
  • Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.

Nice To Haves

  • Associate’s, bachelor’s, or master’s degree in a social service field preferred.
  • If master’s level, work toward clinical licensure.
  • LPC/LCPC or LSW/LCSW preferred.
  • Clinically trained and experience using best practices as well as experience working with a variety of client ages, issues, and symptoms preferred.
  • Familiarity with accreditation and licensure standards preferred.
  • Experience establishing and maintaining collaborative community partnerships preferred.
  • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
  • Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.

Responsibilities

  • Provide whole-person services to clients participating in mental health case management programs.
  • Conduct 1:1 meetings with clients.
  • Provide case management assistance.
  • Utilize evidence-informed interventions targeting behavioral, emotional, and social skill building.
  • Assist clients in achieving their stated goals and objectives.
  • Collaborate with internal and external treatment professionals and community partners.
  • Meet and exceed revenue productivity.
  • Carry a client caseload, as assigned, to meet 60% productivity (24 billable hours per week).
  • Collaborate with clients and/or colleagues to complete lM+CANS, treatment plans, and other required documentation.
  • Understand and follow all compliance rules regarding treatment of services and contracts.
  • Understand and reflect the clinical model of the program, demonstrating principles of Equity, Diversity, and Inclusion.
  • Adhere to all timelines regarding documentation completion.
  • Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individuals in a dignified, respectful, honest, and fair manner.
