About The Position

At Hope Network, over 2,800 compassionate professionals serve individuals across 280+ locations statewide. We are looking for passionate and skilled Case Manager Clinicians to join our team in the West Michigan region. This role involves providing direct therapeutic support, managing care plans, and coordinating services for individuals in need, as part of an interdisciplinary team dedicated to compassionate, person-centered care.

Requirements

  • A Master's degree in Social Work, Psychology, or Counseling is required.
  • Candidates must hold or be in the process of obtaining one of the following licenses: Social Work: Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW); Psychology: Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), or Temporary Limited Licensed Psychologist (TLLP); Counseling: Limited License Counselor (LLC) or Licensed Professional Counselor (LPC).
  • Candidates with a Bachelor’s degree in Social Work may also be considered, provided they are eligible for or in the process of obtaining one of the following credentials: Licensed Bachelor Social Worker (LBSW) or Limited Licensed Bachelor Social Worker (LLBSW), Social Service Technician (SST) Certification.
  • Applicants who are actively pursuing licensure or in the process of applying through LARA (Michigan’s Department of Licensing and Regulatory Affairs) are welcome.

Responsibilities

  • Conduct comprehensive psychosocial assessments.
  • Provide individual and group therapy sessions.
  • Administer, score, and interpret standardized psychological assessments, including measures of intellectual functioning, adaptive behavior, and academic achievement.
  • Coordinate and ensure consumer participation in the Person-Centered Planning (PCP) process.
  • Conduct face-to-face meetings with consumers as medically necessary.
  • Develop and implement Individual Plans of Service (IPOS) tailored to each consumer’s needs.
  • Participate in clinical emergencies, including petitioning and providing testimony during legal proceedings as required.
  • Communicate effectively with internal teams and external partners, including CMH, guardians, and advocates.
  • Facilitate consumer transitions, including intakes, discharges, and transfers.
  • Collaborate with nursing staff to provide behavioral support related to medical appointments, including accompaniment and the collection of relevant health information when needed.
