As a Case Manager Clinician, you'll provide direct therapeutic support, manage care plans, and coordinate services for individuals in need.

Requirements

  • A Master's degree in Social Work, Psychology, or Counseling is required.
  • Candidates must hold or be in the process of obtaining one of the following licenses: Social Work: Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW); Psychology: Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), or Temporary Limited Licensed Psychologist (TLLP); Counseling: Limited License Counselor (LLC) or Licensed Professional Counselor (LPC).
  • Candidates with a Bachelor’s degree in Social Work may also be considered, provided they are eligible for or in the process of obtaining one of the following credentials: Licensed Bachelor Social Worker (LBSW) or Limited Licensed Bachelor Social Worker (LLBSW); Social Service Technician (SST) Certification.
  • We welcome applicants who are actively pursuing licensure or in the process of applying through LARA (Michigan’s Department of Licensing and Regulatory Affairs).

Responsibilities

  • Conduct comprehensive psychosocial assessments.
  • Provide individual and group therapy sessions.
  • Administer, score, and interpret standardized psychological assessments, including measures of intellectual functioning, adaptive behavior, and academic achievement.
  • Coordinate and ensure consumer participation in the Person-Centered Planning (PCP) process.
  • Conduct face-to-face meetings with consumers as medically necessary.
  • Develop and implement Individual Plans of Service (IPOS) tailored to each consumer’s needs.
  • Participate in clinical emergencies, including petitioning and providing testimony during legal proceedings as required.
  • Communicate effectively with internal teams and external partners, including CMH, guardians, and advocates.
  • Facilitate consumer transitions, including intakes, discharges, and transfers.
  • Collaborate with nursing staff to provide behavioral support related to medical appointments, including accompaniment and the collection of relevant health information when needed.
