About The Position

We are seeking a Junior Clinician / Case Manager to support a high-impact healthcare engagement. In this role, you will work alongside multidisciplinary clinical teams to coordinate patient care, drive effective care transitions, and ensure appropriate utilization of healthcare resources.

Requirements

• Active RN or licensed clinical professional (LPN, LCSW, or equivalent)
• 1-3 years of experience in case management, utilization review, or care coordination
• Familiarity with discharge planning processes and post-acute care settings
• Experience working with managed care or third-party payer requirements
• Strong communication and organizational skills

Nice To Haves

• Experience in a hospital or health system environment
• CCM (Certified Case Manager) certification is a plus
• Knowledge of InterQual or Milliman criteria

Responsibilities

• Plan and coordinate patient care activities in collaboration with physicians, nurses, and multidisciplinary care teams
• Conduct clinical assessments and actively support care planning and implementation
• Ensure patients receive the appropriate level of care throughout their healthcare journey
• Perform timely and effective discharge planning to support smooth post-discharge transitions
• Collaborate with third-party payers to verify and ensure coverage for patient services
• Maintain accurate and compliant documentation in accordance with regulatory standards
