About The Position

In the challenging yet rewarding role of a Case Manager, you will facilitate a strategic planning method called the Child and Family Team process. This process supports youths with complex behavioral or emotional needs in achieving their optimum level of wellness through the development of individualized service plans, linkage to community resources, partnership with family members and others involved in their lives, and the coordination of behavioral and physical health care. Southwest Network provides integrated behavioral and physical health services, caring for both the mind and body for whole health and wellness. We provide treatment programs for seriously mentally ill adults and children and adolescents with complex emotional and behavioral needs. Requirements

  • Master's or bachelor's degree with a behavioral health or related field OR Bachelor's degree in a non-related field PLUS one year related experience OR Associate's degree in any field PLUS one year related experience OR High school diploma or equivalency certificate PLUS three years' related experience
  • Maintain current CPR, First Aid, and Therapeutic Options certifications.
  • Maintain a valid Arizona state fingerprint clearance card.
  • Maintain a valid Arizona driver's license and car insurance.
  • Must be at least 21 years of age.
  • Ability to maintain excellent member and family relationships and provide excellent customer service.
  • Effective written, verbal and interpersonal communication skills.
  • Ability to stay organized.
  • Critical thinking, time management and problem solving skills.
  • Ability to utilize multiple types of office equipment (i.e. phones, copier, fax, etc.).
  • Proficient in Microsoft Office software.
  • Able to utilize the internet and various web browsers.

Nice To Haves

  • Ability to type 25 to 40 WPM is preferred.
