This position initiates, facilitates, and performs a variety of casework related functions pertaining to children residing in the children’s residential program. The Case Manager carries a caseload of children living in assigned homes, ensuring appropriate services are implemented and collaborating with staff for the child’s overall care, progress, and treatment. Responsibilities include securing documentation, compiling historical information, assessing case situations, and collaborating with an interdisciplinary team to develop and implement treatment plans. The role also involves attending IEPs, advocating for children, monitoring placements, and responsible for permanency and discharge planning. Maintaining contact with parents/guardians and working with DCFS are key aspects, as is ensuring compliance with licensing standards and laws. The Case Manager also assists with the intake process for new children and facilitates staffings and attends required meetings and trainings.
