About The Position

The Bilingual Case Manager provides compassionate, person-centered support to individuals facing barriers related to housing, health, benefits, and community integration. This role focuses on empowering people to identify their goals, access resources, navigate complex systems, and make informed decisions that promote independence and well-being. Through outreach, advocacy, and ongoing support, the Case Manager helps individuals build pathways toward greater stability and self-sufficiency.

Requirements

  • Bi-lingual, English/Spanish.
  • Experience working in social services, peer support, case management, or community-based programs serving individuals with diverse needs.
  • A person-centered and non-judgmental approach, with the ability to build rapport and meet individuals where they are.
  • Strong organizational and time-management skills, with the ability to manage multiple priorities while maintaining attention to detail.
  • Knowledge of, or the ability to quickly learn, public benefits systems, housing resources, and community-based services.
  • Excellent interpersonal, problem-solving, and advocacy skills.
  • A valid, unrestricted driver's license and the ability to travel throughout the community as needed.

Nice To Haves

  • A Bachelor's degree in Human Services, Social Work, Psychology, Sociology, or a related field.
  • Three (3) or more years of experience supporting individuals with disabilities, behavioral health needs, or other complex circumstances.
  • Knowledge of Orange County community resources, service providers, and referral networks.
  • Experience with crisis intervention, motivational interviewing, or trauma-informed practices.
  • Familiarity with electronic case management systems and documentation standards.

Responsibilities

  • Develop meaningful relationships with individuals and assist them in identifying goals and creating personalized plans that support housing stability and independent living.
  • Connect individuals to community resources and services, including healthcare, employment, transportation, food assistance, and public benefits.
  • Guide individuals through complex systems and processes by providing education, advocacy, coaching, and practical support.
  • Conduct outreach and engagement activities to connect with individuals who may be hesitant or difficult to reach, building trust through persistence and empathy.
  • Promote informed decision-making by providing clear, accessible information and respecting each individual's right to self-direction.
  • Monitor changing needs, proactively address potential challenges, and respond appropriately during times of crisis.
  • Maintain accurate, timely documentation and records to track progress, ensure compliance, and support continuous quality improvement.
  • Participate actively in team meetings, case consultations, and program planning activities, contributing insights and recommendations to enhance services.
  • Provide services and communication in both English and the required second language to ensure equitable access and understanding.
