About The Position

Capital Health is seeking a full-time Case Manager for the Department of Behavioral Health. This role involves conducting patient intakes, assessing patient needs, developing treatment plans, and coordinating care with patients, families, and the interdisciplinary treatment team. The Case Manager will document patient information, refer patients to appropriate services, and advocate for patient needs. This position requires a Bachelor's degree in a human services-related field and experience in a clinical setting, preferably in mental health or a hospital environment. The role involves direct patient interaction and requires strong communication and documentation skills.

Requirements

  • Bachelor’s degree in human services related field.
  • Minimum two years of experience in a clinical setting preferred.
  • One year experience in a clinical mental health setting or in a hospital-based setting.

Responsibilities

  • Conducts intakes for all patients interested in scheduling appointments for the Department of Behavioral Health.
  • Identifies patient’s problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with patient, family, and interdisciplinary treatment team.
  • Documents medical and mental health diagnoses, disabilities and treatment procedures, to include acute, chronic and traumatic illnesses and injuries, and common medications in patient chart.
  • Uses culturally competent skills to assess individual patient needs and refers to appropriate professional staff when necessary.
  • Identifies barriers and plans for intervention to overcome or lessen barriers to achieve outcome as evidenced by treatment plan.
  • Documents clinical record.
  • Assesses comprehensively patient needs including clinical interventions, support, and appropriate community resources according to policy.
  • Represents patient’s needs and rights to providers and community in a variety of settings.
  • Completes forms and letters necessary to support patients’ social service needs in collaboration with providers.
  • Provides crisis assessments and crisis interventions in collaboration with clinical team as needed.
  • Identifies and maintains collaboration with community support programs to address the comprehensive care needs of patients.
  • Responsible for maintaining and developing patient education binder/materials.
  • Actively participates in clinical team meetings.
  • Performs other duties as required.
