About The Position

This role is for a Case Manager Assistant within the SOAR program. The position is office-based with a schedule that may become irregular if emergencies arise. The assistant will collaborate with community partners, conduct interviews with individuals and clients, gather medical records, and complete SSI/SSDI applications. Responsibilities include assessing eligibility, initiating paperwork, summarizing disability documentation, completing SOAR medical summary reports, and maintaining client records in accordance with Florida Statutes and Administrative Codes. The role also involves completing all necessary documentation accurately and within required timeframes, such as progress notes, time cards, and client opening procedures in Avatar. The assistant is expected to achieve individual goals and function as a team player, demonstrating dignity, respect, and professionalism in all interactions.

Requirements

  • High School Graduate or GED Certificate required.
  • Able to read, write and comprehend oral instructions.
  • Willing to use or learn computer software.
  • Extensive clerical experience.
  • Computer and typing skills that reflect accuracy and speed.
  • Must be flexible, well organized and able to handle multiple, simultaneous tasks.
  • Able to make independent decisions, prioritize and meet multiple, simultaneous deadlines.
  • Must be 21 years of age.
  • Negative Drug Screening prior to hire and throughout employment.
  • Clear Criminal Background Check throughout employment with Suncoast Center and local checks prior to hire and annually along with DCF Level II prior to hire and as required by DCF.
  • Must have valid Florida Driver's License.
  • Motor Vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment.
  • Must have reliable vehicle, valid registration, and valid vehicle insurance.
  • Must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character.
  • Computer literate.
  • Must drive personal vehicle and carry agency requires minimum auto insurance.
  • All positions require a screening through the Clearinghouse.

Responsibilities

  • Collaborate with community partners to identify applicants, complete interviews with individuals and gather medical records pertinent to the application process.
  • Complete interviews with clients to gather information to complete SSI/SSDI applications.
  • Assess clients' eligibility for SOAR services by collecting and summarizing disability documentation, and submitting final applications on behalf of clients.
  • Initiate paperwork with clients as referred to program by filing initial documentation of representation with SSA office.
  • Gather medical records and other information to complete SSI/SSDI applications.
  • Complete SOAR medical summary reports for client applications.
  • Complete SOAR training and attend any additional trainings/certifications as required by the state.
  • Completes all documentation to open the client into the Adult Case Management episode in Avatar.
  • Maintains a Continuity of Care in accordance with Florida Statutes and Florida Administrative Codes.
  • Completes all other documentation correctly and in the required time frame including progress notes with objective to treatment plan clearly stated, time cards, and other documentation as requested by the Program Manager needed to ensure all client records are complete and accurate.
