About The Position

Weber Human Services (WHS), located in Ogden, Utah serves a growing urban population residing in Weber and Morgan Counties. WHS is an organization dedicated to helping those in need rebuild their lives and reach their full potential. WHS was organized by a cooperative agreement between the Weber and Morgan county governments to serve as the local authority for the provision of their substance abuse, mental health and senior service programs. Weber Human Services is the only organization in the State of Utah providing these services under a single administration. Through dozens of human services programs, WHS helps thousands of its neighbors by supporting and empowering our most vulnerable groups including at-risk youth, the elderly, children, youth and adults suffering from mental illnesses, and those recovering from addictions. Requirements

  • A bachelor's degree in social work or related field
  • At least one year of working with the elderly population or younger people with disabilities
  • Valid Utah driver license
  • Basic computer skills

  • An active SSW (Social Service Worker) license

Responsibilities

  • Be a mediator for residents in long-term facilities in Weber and Morgan county, such as nursing homes and assisted living facilities, by working with or on behalf of residents to resolve complaints and concerns.
  • Inform and educate residents, families, facility employees, and visitors about residents' rights, community services, laws, regulations and standards that govern long-term facilities.
  • Participate in occasional and periodic team meetings, community events and related training.
  • Help individuals navigate the Medicare system through one-on-one counseling and outreach events including submitting Medicaid applications.
  • Educate seniors about health care fraud and abuse.
  • Provide assessment, care planning and case management of provider-delivered services for individuals who are eligible for in home services programs in order to support the qualified clients to stay safely and independently in their own residence as much as possible.
