About The Position

Davis Behavioral Health, Inc. is actively recruiting a full-time Case Manager to work in the Adult Mental Health program on the FAST (Community Outreach)/AOT (Involuntary Commitment) team. This position will be based out of the DBH Layton office; however, the majority of the work will be conducted in the community throughout Davis County. The FAST/AOT Case Manager provides client-centered, trauma-informed, and ethically grounded case management services to individuals in Davis County who qualify as classified as seriously and pervasively mentally ill by a qualifying diagnosis and a combination of other criteria including the need for medications management, inability to perform activities of daily living, are marginally employed or unable to be employed without support, are socially isolated, are on public assistance programs. Some of these clients may have been court-ordered to receive mental health treatment under civil commitment. This role supports clients' engagement with treatment and promotes recovery, autonomy, and community integration. The Case Manager utilizes the Assertive Community Treatment (ACT) model of care to bring hospital level of care and coordination to the client in the community. The case manager facilitates access to needed services, and advocates for clients' rights and dignity throughout their recovery process to reduce the frequency and duration of incarceration, hospitalization and county crisis services. The position requires demonstrated cultural humility, clinical judgment, strong interpersonal skills, and the ability to form collaborative relationships with clients, natural supports, legal entities, and multidisciplinary providers.

Requirements

  • Graduation from a college or university with a bachelor's degree in social work (BSW) AND active licensure as a Social Service Worker (SSW) in good standing in the State of Utah.
  • Candidates who meet qualifications to obtain SSW licensure within 6 months of employment may also be considered.
  • The FAST/AOT Case Manager must complete and maintain certification in First Aid and CPR, Daily Living Assessment (DLA), and all mandatory training assigned to this position.
  • Position may be under-filled by an individual who has graduated from a college or university with a bachelor's degree in another social science field (i.e., psychology, criminal justice, sociology, etc.) AND has active licensure as a Social Service Worker (SSW) in good standing in the State of Utah or can meet criteria to obtain licensure within 12 months of hire.
  • The ideal candidate will have a working knowledge of mental health and substance use disorder services, community resources, experience working with clients that have severe mental illness, is hardworking, and can work independently as well as in a team environment.
  • Experience in mental health case management or related behavioral health services preferred (1–2 years).
  • Demonstrated cultural competence and commitment to ethical social work practice in accordance with the NASW Code of Ethics.
  • Ability to establish and maintain professional boundaries with empathy and respect.
  • Effective written and verbal communication skills.
  • Strong crisis de-escalation, problem-solving, and advocacy skills.
  • Technological proficiency to complete documentation in the field and communicate with team members securely.
  • Must be at least 18 years old with a valid driver's license and clean driving record
  • All employees at DBH must successfully pass a pre-employment criminal background check as well as a pre-employment drug screening.
  • Candidates who are offered employment are required to report for a drug screen at an Intermountain WorkMed location within 24 hours of being requested to do so by the DBH Human Resources Department.
  • Davis Behavioral Health works with Federal grants and must comply with the Federal drug laws, including no use of medical marijuana.
  • All employees at DBH must successfully pass a pre-employment criminal background check as well as a pre-employment drug screening.
  • Failure to pass either screening will result in the termination of any job offer submitted and/or employment for any individual who has started employment while the criminal background screening is in process.
  • Pending and/or open charges will likely impact a candidate's ability to pass the pre-employment criminal background screening.

Responsibilities

  • Provide case management using the generalist intervention model: engagement, assessment, planning, implementation, evaluation, termination, and follow-up.
  • Monitor compliance with treatment plans and coordinate care with internal teams and community providers.
  • Build and maintain therapeutic rapport with clients, respecting their self-determination while promoting engagement in services and substance use and mental health recovery.
  • Assist clients in accessing resources including but not limited to medical, dental, legal, financial, housing, and transportation services.
  • Educate clients and their families or other natural supports about substance use, mental illness and how to support the client in recovery and maintenance.
  • Provide skill-building support to increase independence in activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Conduct outreach and engagement with clients who are not actively participating in services or who have recently shown an increase in symptoms or decrease in wellness.
  • Participate in multidisciplinary care planning and coordinate treatment needs with intake, psychiatry, and therapy teams.
  • Maintain timely, accurate documentation in compliance with agency and licensing standards (within 24 hours).
  • Safely operate a company vehicle and transport clients to and from appointments when needed.
  • Meet both monthly and annual productivity standards for direct service.
  • Participate in required training and maintain position specific certifications, including but not limited to CPR/AED and administration of Naloxone, case management trainings, daily living assessment skills and maintaining SSW licensure or TCM certification including attending to the CEU requirements.
  • Carry out other duties as assigned in support of client care and program goals.
  • Outreaching clients who are not attending appointments or are dodging treatment
  • Coordinating client needs and status with other DBH units.
  • Ability to build professional relationships with community partners
  • Coordinating physical outreach of clients not engaged in treatment
  • Building multi-disciplinary teams to support client wellness and recovery
  • Addressing emergent needs as soon as possible once identified
  • Monitoring and responding to company communications in a timely manner through Email/TEAMS/Text in accordance with HIPPA regulations
