Case Manager - Adult Mental Health
About The Position
Make a meaningful difference in the lives of adults living with a mental health illness. Hampton-Newport News Community Services Board (H-NNCSB) is hiring a full-time Adult Mental Health Case Manager to provide behavioral health case management, care coordination, advocacy, and supportive services to adults in the Hampton and Newport News communities. If you are a Bachelor's-level human services professional with case management experience who is passionate about connecting people to resources, supporting treatment goals, and helping individuals achieve greater stability and independence, we want to hear from you.
Requirements
- Bachelor's degree in a Human Services field (such as Social Work, Psychology, Sociology, Counseling)
- One (1) year of case management experience
- Must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
Responsibilities
- Assess client needs
- Coordinate services
- Develop and monitor treatment plans
- Connect individuals with community resources
- Provide supportive counseling and advocacy
- Respond appropriately to urgent or crisis situations
