About The Position

The ACT Criminal Justice Rehabilitative Case Manager (RCM) position will work directly with the Galveston County jail to identify individuals who qualify for ACT services and require specialized, intensive support to avoid jail recidivism. The RCM will manage a caseload of 10 individuals with mental illness who are high-utilizers of the Galveston County jail, providing enhanced engagement and specialized ACT-intensity case management and rehabilitative services. The target population includes individuals eligible for ACT services who have had two or more bookings to the Galveston County jail in the past year. This role also supports current ACT clients who become involved with the justice system, addressing their specific needs to prevent arrest. ACT is an integrated service model that combines clinical and rehabilitation expertise within a mobile team, working in partnership with individuals in recovery from their homes. ACT teams offer comprehensive psychiatric treatment, rehabilitation, and support to individuals with serious and persistent mental illness, enabling them to live independently in the community. The objective is to provide intensive services to reduce recidivism and increase community tenure. Through intensive mental health services, individuals will receive frequent contact to facilitate lasting engagement, linkage to housing, and benefits assistance using an integrated team approach to decrease re-entry into the correctional system. The ACT Criminal Justice RCM will also assist jail referrals who do not qualify for ACT services by referring them to other appropriate resources. Requirements

  • Requires a bachelor's degree or equivalent in the behavioral sciences field of study from an accredited college or university and/or specialized QMHP certification.
  • Minimum one year related work experience with behavioral health services.

Responsibilities

  • Delivers services in accordance with organizational service delivery standards, maintaining a minimum of 50% of time worked in direct service provision (with a ramp-up period for new hires).
  • Completes accurate, collaborative documentation that is entered and electronically signed within 24 business hours.
  • Identifies a current and discreet caseload as part of an interdisciplinary team.
  • Serves as a member of the treatment team, coordinating and providing services as needed.
  • Actively participates in team meetings.
  • Displays professionalism and team membership when representing the program.
  • Maintains daily contact with designated staff regarding current work area and message retrieval.
  • Provides RCM self-management strategies and clinical coordination to individuals, groups, and family members using evidence-based strategies within the (TRR) model of care.
  • Provides crisis services to consumers and walk-ins to the clinic as needed.
  • Provides crisis services for after-hours response, including nights, weekends, and holidays, for ACT clients on a rotating basis.
  • Provides monthly documented face-to-face direct service hours to GCC consumers and/or family members according to the current Direct Service Agreement.
  • Monitors consumer requirements within the Disease Management Model to ensure required services are offered, meeting at least 80% service hours for the identified caseload.
  • Assures treatment provided to consumers relates to the individualized goals of the treatment plan.
  • Utilizes a variety of skills training in conjunction with the IMR (Illness Management and Recovery Model) and may use intervention strategies such as instruction, positive feedback, repetition, role play, behavioral reversal, in-vivo practice, and coaching.
  • Follows up on potential ACT qualifying referrals within 2 business days to determine eligibility.
  • Increases enrollments of ACT eligible high-utilizers of the Galveston County jail.
  • Serves an average of 10 unduplicated and 70 duplicated individuals per month, and 20 total unduplicated individuals for the fiscal year, with the expected outcome of reduced jail recidivism for ACT-served individuals.
  • Maintains compliance with all documentation for consumers as assigned and required by GCC policies and procedures.
  • Conducts the Adult Needs and Strengths Assessment (ANSA) every 6 months or as clinically indicated for assigned individuals.
  • Ensures the completion of the initial and annual ANSA recertification pursuant to the Praed Foundation and HHSC contract.
  • Participates in the Medicaid Administrative Claiming process.
  • Assists with tracking vital health indicators as part of CCBHC & 1115 Waiver outcome measures.
  • Adheres to and promotes the agency compliance policy and collaborates with various departments to foster and develop community partnerships.
  • Treats all individuals with courtesy, decency, and respect.
  • Provides responsive and efficient services.
  • Interacts with staff, consumers, local entities, and advocacy groups to meet local needs.
  • Provides the highest level of customer service.
  • Reports personnel leave, flex time, and emergency leave to the assigned supervisor.
  • Adheres to professional, ethical, and legal requirements set forth in GCC's Business Code of Conduct and Corporate Compliance Plan.

  • May be required to travel within Brazoria and Galveston Counties.
  • Out of region travel may be required to attend trainings.

  • Assists supervisor when requested to complete special projects and assignments within a negotiated timeframe.
  • Completes other duties as assigned by supervisor as it relates to the program and position.
