Case Manager 2 - SHP (Bridgeport)
About The Position
The Supportive Housing Program (SHP) of Fairfield works with chronically homeless women and families in Bridgeport, CT. The program offers permanent rent subsidy and intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care, the program partners with program participants to identify goals and resources that help them achieve long-term stability. The Case Manager 2 is responsible for providing home- and community-based case management services to homeless women and families throughout Bridgeport, CT. Working within our Homeless Services, the Case Manager 2 works with the SHP program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.
Requirements
- Associate’s Degree
- 2+ years relevant experience in direct service and case management field
- Valid Driver’s License
- Regular and predictable attendance is required
Responsibilities
- Delivering comprehensive case management services to clients in their homes and in their communities
- Conducting intake assessments
- Assisting youth in locating available apartments for rent
- Working in partnership with participants to develop and implement individual service plans that will progress them to stable housing
- Liaison with treatment providers and community resources
- Providing transportation to participants as needed
- Developing money management skills with caseload
- Developing employment readiness skills with caseload
- Liaison with landlords and property management staff to support participant's continued tenancy
- Data entry and documentation
