About The Position

The Intake and Eligibility Program is the gateway to Individual Development Disabilities Services. Eligibility Specialists conduct both new eligibility determination and redeterminations in accordance with OAR 311-320-0080. This position will support Eligibility Specialists with connecting with applicants who are hard to reach at any point of the determination process and those who need referrals to immediate community resources support. This position will also conduct outreach to clients who are hard to reach during the redetermination of eligibility process thus retaining clients enrolled in the Individual Developmental Disabilities program. This is a KSA (Knowledge Skills and Abilities) position. Must be fluent in Spanish (speak, read, and write). Language testing is required for this position.

Requirements

• A case manager must have knowledge of the public system for developmental disabilities services in Oregon and at least one of the following: A bachelor’s degree in behavioral science, social science, or a closely related field. A bachelor’s degree in any field and one year of human services related experience, such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing. An associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging disabilities, prevention, health, cultural competencies, or housing. Three years of human services related experience, such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing.
• Must be fluent in Spanish (speak, read, and write).
• Must pass a criminal background check.
• Must have a valid driver license.

Responsibilities

• Maintain an ongoing caseload of individuals and families who are newly applying for I/DD services or are in the eligibility redetermination process.
• Complete outreach and collaborate with community members including but not limited to DHS Child Welfare, Adult Protective Services, Behavioral Health, Oregon State Hospital, Oregon Corrections, Oregon Youth Authority, Hospital Emergency Services.
• Assist with intake interviews for children.
• Assist in intake interviews for children under guardianship of DHS.
• Assist in completing redetermination interviews and outreach for enrolled clients entering school age and adult redeterminations and inform them of criteria per the I/DD Oregon Administrative Rules.
• Provide primary staffing for weekly in person drop-in sessions for applicants to complete paperwork.
• Document interactions with clients/legal representatives via progress notes system.
• Complete home and community visits to assist applicants and clients to complete eligibility paperwork.
• Process incoming paperwork by reviewing for completion, uploading to applicant/redetermination file.
• Research and Identify community resources to provide applicants and enrolled clients with immediate support needs.
• Identify clients who may benefit from other services not available from I/DD and make appropriate referrals to community resources.
• Bring questions related to client services and access to weekly team and case consult meetings.
