About The Position

Responsible for collaborating with the medical team to develop and implement a plan of care which coordinates all of the required services. Must develop a knowledge and working relationship with outside social service agencies to ensure patients have access to all eligible programs.

Requirements

• Bachelor's degree in Social Work, Psychology, Psychiatric Rehabilitation or mental health related discipline.
• Ability to counsel and educate consumers and their families.
• Ability to resolve problems that require the use of basic mental health knowledge.

Nice To Haves

• Bilingual (Spanish/English) preferred.
