About The Position

Under the supervision of the Ryan White Program Manager, the Case Management Supervisor is responsible for supporting and overseeing a team of case managers delivering services that lead to satisfactory health outcomes for people receiving community health center services as well as for people living with HIV/AIDS in AHC’s service area people living with HIV/AIDS in Affinity Health Center’s (AHC) service area.

  • Experience coordinating federal and state benefits and/or assistance (e.g., Medicaid/Medicare, SNAP, SSDI, SSI, etc.)
  • Experience guiding patients through Affordable Care Act (ACA) Marketplace, Medicaid/Medicare enrollment, or other (i.e., providing education on eligibility, options, timelines, supporting application, etc.)
  • Experience with electronic health records
  • Bachelor’s degree in public health, social work or other related field
  • Experience working in Ryan White Funded programming
  • Experience working with HIV care management software(s) for Ryan White funded programs (i.e., CAREWare and/or Provide Enterprise)
  • Valid Driver’s license and reliable transportation

Responsibilities

  • Recruit, onboard and train new Medical Case Managers (MCM)
  • Coordinate ongoing routine trainings for MCM team (e.g., RWB MCM training, ACA open enrollment or Medicaid eligibility training, etc.)
  • Identify and coordinate opportunities for staff professional development
  • Review initial program intake assessments and annual reassessments
  • Conduct chart audits in Provide Enterprise and electronic health record (EHR) to ensure accuracy and compliance with program eligibility
  • Contribute to annual budgeting process
  • Partner with Accounting Team, to identify denied claims for patients who are eligible for assistance under the SC AIDS Drug Assistance Program (ADAP). Determine gaps or discrepancies in patient files and documentation that led to the denied claims and work with MCM staff to address these gaps.
  • Work with Data Specialists to monitor monthly reports and dashboards.
  • Review service data and quality metrics in Provide Enterprise and Azara to identify opportunities for quality improvement across case management performance
  • Contribute to monthly/quarterly/annual reports to local, state and federal agencies, including the annual RSR
  • Prepare for annual audit site visits as required by funders, ensure documentation is up to date, fully compliant
  • Attend funding recipient cohort meetings as assigned (e.g., SC DPH RWB MCM Workgroup Meeting, etc.)
  • Build and maintain relationships with referral, peer and partner organizations.
  • Maintain list of external service providers, support agencies and resource programs.
  • Ensure all MCM services are delivered in accordance with the SC Ryan White Part B Case Management and Ryan White Part A guidelines and AHC policy and procedure
  • Participate in Continuous Quality Improvement (CQI) activities
  • Participate in AHC Performance Management System
  • As a member of the AHC leadership team, lead with compassion, curiosity and courage
