About The Position

This position is responsible for interviewing, accepting, and providing comprehensive, long-term, structured, complex case management services for an assigned caseload of clients in an established life management program. Develops program plan/goals and evaluates client’s progress by conducting mentoring and counseling sessions with client and/or staff.

Requirements

  • Bachelor’s degree or greater from an accredited college or university in Social Work, Behavioral Science, or a related field AND Three years progressively responsible experience providing direct case management social services or Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
  • Valid Driver’s License.

Responsibilities

  • Interviews and conducts needs assessment to determine eligibility for program based on guidelines and requirements.
  • Make referrals to other agencies if not appropriate for program.
  • Prepares and maintains case records of assigned clients.
  • Enters pertinent information into the Homeless Management Information System (HMIS).
  • Establishes comprehensive goals for eligible clients
  • May accompany and/or provide transportation for clients to meetings related to case plan.
  • Plans, coordinates and/or facilitates life management, behavior modification, etc. classes to assist clients.
