About The Position

This position is responsible for interviewing, accepting, and providing comprehensive, long-term structured, complex, case management services for an assigned caseload of clients participating in a housing shelter program. The role involves providing coaching hours to promote the creation and implementation of individualized housing plans by clients. The specialist must understand the uniqueness of each client's history to determine the most effective program plans, advocate for clients, and promote positive outcomes. This position will service clients in a specialized Veterans program and a secondary emergency shelter program.

Requirements

Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field.
Three years progressively responsible experience providing direct case management social services including accessing clients' needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources.
Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
Valid State Driver's License.

Responsibilities

Establishes goals with clients to promote stability and the securing of stable housing.
Communicates with clients in a manner that is easily understood.
Completes progress notes and modifies goals, as needed.
Refers clients to additional professional counseling resources, if needed.
Maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.
Meets regularly with clients to discuss and evaluate their progress.
Prepares accurate and up-to-date records documenting the same.
Completes incident reports, receives relevant incident reports and enforces policies and procedures, including but not limited to behavior and compliance plans.
