About The Position

This position is responsible for providing comprehensive, long-term, structured, complex case management services for an assigned caseload of clients in an established life management program. The role involves developing program plans and goals, and evaluating client progress through mentoring and counseling sessions.

Requirements

  • Bachelor’s degree or greater from an accredited college or university in Social Work, Behavioral Science, or a related field.
  • Three years progressively responsible experience providing direct case management social services.
  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
  • Valid Driver’s License.

Responsibilities

  • Interviews and conducts needs assessment to determine eligibility for program based on guidelines and requirements.
  • Make referrals to other agencies if not appropriate for program.
  • Prepares and maintains case records of assigned clients.
  • Enters pertinent information into the Homeless Management Information System (HMIS).
  • Establishes comprehensive goals for eligible clients.
  • May accompany and/or provide transportation for clients to meetings related to case plan.
  • Plans, coordinates and/or facilitates life management, behavior modification, etc. classes to assist clients.
