The Case Management Specialist provides case management services within the SSVF program. This role involves coordinating various aspects of case management, including referral, intake, eligibility determination, program planning, monitoring, assessment, and evaluation of clients. The specialist will monitor and record client progress, ensure files are maintained appropriately, and build professional relationships with organizations, county personnel, families, and service providers. Responsibilities also include directing case service allocations, participating in committees, collecting performance measure data, conducting follow-up with exited members, making decisions based on program operations and fiscal analysis, understanding safety precautions, traveling as required, articulating program guidelines, making recommendations on work methods, assisting with work plans, managing the program within State/Federal regulations, and maintaining professional expertise through education and attendance at relevant events. The role also involves healthcare navigation.
