About The Position

The Case Management Nurse is responsible for the patient's journey, ensuring their medical, social, and spiritual needs are met from admission until they safely transition home or to another facility. This role involves identifying and removing barriers to prevent readmissions, such as lack of insurance, transportation, or home support. The nurse also serves as a liaison between the clinical team and payers, ensuring accurate documentation for reimbursement. This position emphasizes mission-driven service with Reverence and Integrity, treating discharge planning as a ministry of hope and dignity for vulnerable populations.

Requirements

Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
