Requirements

Current Massachusetts Registered Nurse (RN) license required.
Valid driver's license, reliable transportation, and a good driving record required.
Strong communication, interpersonal, and organizational skills.
Ability to work independently and manage priorities effectively.
Demonstrated reliability, professionalism, and respect for diverse populations.

Nice To Haves

Minimum of 2 years of human services experience preferred.

Responsibilities

Conduct clinical eligibility assessments for Adult Foster Care members.
Complete 1–2 home visits per day.
Attend required in-person meetings and home visits.
Complete prior authorizations and required documentation.
Ensure compliance with program standards and contractual requirements.
Work independently while collaborating with an interdisciplinary team.
Assist with program organization, expansion, and staff training as needed.
